I hereby give permission to use any football related photography of my son for DCO publication. I hereby authorize the Deer Creek Optimist to act for me according to their best judgment in an emergency requiring medical attention. I hereby waive and release Deer Creek Optimist and all its coaches and players from liability. I know of no mental or physical problems which may affect my child's ability to safely participate in football. I will be responsible for any medical and other charges in connection with DCO. I will obey and encourage my child to obey the rules and regulations laid down by DCO. I will also encourage and teach by my example good sportsmanship to my child and those I come in contact with during league activities.
